Abstract:

Backgroung: Bridelia ndellensis (Euphorbiaceae) is used as a traditional plant treatment of diabetes. The aim of the present study was to investigate the mechanism involved in the antihyperglycemic effects of this plant. Methods: An intestinal perfusion technique was used to study the effect of the ethanol extract of B. ndellensis on the intestinal absorption of glucose in normal rats fasted for 36 h. The effect of the extract (50 and 100 mg/kg) on Insulin secretion was evaluated using BRIN-BD11 pancreatic cells. Results: In a perfused rat intestinal preparation in situ, the plant extract at a concentration of 5 mg/ml did not show any inhibitory effects on glucose absorption when compared with control animals. Ouabain which is an inhibitor of glucose transport across the small intestine significantly (p < 0.001) decreased the glucose absorption during the whole perfusion period compared with the control group. On the other hand, the plant extract inhibited the gastrointestinal motility induced by Barium sulphate milk in rats. The ethanol extract of B. ndellensis at concentrations of 50 and100mg/ml stimulated in a concentration-dependent manner the insulin secretion from BRIN-BD11 cells in vitro. The induction of insulin release was significantly different when compared to control with the concentration of 100 μg/ml (P < 0.001). Abstract:
This paper presents a new technique for expressing rhotrices in a generalize form. The method involves using multiple array indexes as analogous to matrix expressions, unlike the earlier method in the literature, which can only be functional in a single array computational environment. The new rhotrix look will encourage the study of rhotrix algebra and analysis from a better perspective. In addition, computing efficiency and accuracy will also be improved, particularly when the operations in rhotrix space over the new expression are algorithmatized for computing machines.
Reimplantation of Accidentally Avulsed Permanent Maxillary Anterior Teeth in a 10-Year-Old Girl

Abstract: Accidental tooth avulsion is a grievous injury and common among the children. Management of avulsed tooth within alveolar socket by reimplantation becomes a challenge for the clinician due to extraoral time and media of transportation. Although the long-term prognosis of reimplantation is poor, the time during which the tooth remain within the arch will guide the development of alveolar bone completely. Moreover, reimplantation will maintain anatomical, functional and esthetic rehabilitation of the patient. In this case report, we present a case of accidental avulsion where teeth were gently rinsed of any debris and placed in normal saline during the examination and preparation of the reimplantation sockets. The teeth were then reimplanted, functionally splinted followed by endodontical treatment. After 12 months follow-up the periodontal space was healed perfectly without any resorption or ankylosis. Abstract: MRSA is able to generate a modified variety of penicillin binding protein named as PBP2a instead of PBP which makes it resistant against penicillin and methicillin. Production of PBP2a is due to the presence of a gene on Staphylococcal cassette chromosome or SCC termed as “mec-A gene”. SCC is a mobile genetic element carrying many resistance genes. It is because of current antibiotic selection pressure that by now there are eight types (I - VIII) of SCCmec. This research has been designed to determine frequency of SCCmec type IV and V in clinical isolates of MRSA. A total of 70 presumptive MRSA isolates collected from a tertiary care hospital of Lahore were cultured on blood agar, incubated overnight at 37°C aerobically. Next day, they were examined for cultural characteristics, colonial morphology, gram stain, and biochemical profile. Confirmation of MRSA was done by phenotypic disk diffusion method according to (CLSI) 2013 guidelines. mecA gene was also detected at molecular level. Molecular identification of SCCmec type IV and V was done by Nested PCR strategy. A total of 50 isolates were confirmed to be MRSA Molecular detection of SCCmec type IV and V revealed that 11 isolates (22%) possess SCCmec type IV and only 2 isolate (4%) carries SCCmec type V. It is obvious from results that SCCmec type IV and V are present in our population too. Larger study (with larger sample size) might be undertaken to find out actual emergence of SCCmec type IV and V in our population.
Cardiometabolic Risk in Overweight and Obese Children in Bangladesh  [PDF]

Abstract: Introduction: Childhood obesity is increasing dramatically and represents an important public health issue due to associated metabolic and cardiovascular comorbidities. Very limited data are available regarding cardiometabolic risk factors among this group in Bangladesh. Objective: To observe the cardiometabolic risk factors in overweight and obese children. Methods: This cross-sectional study was carried out in 88 overweight and obese children recruited consecutively by using CDC percentile chart for body mass index (BMI) in children between January 2017 and March 2018 in the Department of Endocrinology, Bangabandhu Sheikh Mujib Medical University, Dhaka, Bangladesh. After completing a questionnaire and relevant clinical examination, blood was collected for fasting plasma glucose (FPG), insulin, HbA1c, lipid profile and C-reactive protein (CRP). Homeostasis Model Assessment of Insulin Resistance (HOMA-IR) was used to determine insulin resistance. Results: Central obesity (100%), dyslipidaemia (88.6%), raised CRP (81.8%) and metabolic syndrome (69.3%) were the most common cardiometabolic risk factors. Children with grade 3 obesity had significantly higher systolic blood pressure (115.57 ± 11.60 vs 105.71 ± 8.84 mmHg, p = 0.043) and insulin resistance (7.15 ± 4.97 vs 3.53 ± 2.04, p = 0.017) than grade 1 obesity. Blood pressure, insulin resistance and CRP increased while high density lipoprotein (HDL) decreased with increasing severity of obesity. BMI z score was a significant predictor of systolic blood pressure; waist circumference was an independent predictor of diastolic blood pressure and HDL; waist height ratio best predicted insulin resistance, CRP and total cholesterol in overweight/obese children. Conclusions: We have observed a high frequency of cardiometabolic risk factors in overweight and obese children and they increased worsened with increasing grade of obesity.

Abstract: Heart disease is a major cause of morbidity and mortality in modern society. Medical diagnosis is extremely important but complicated task that should be performed accurately and efficiently. Although significant progress has been made in the diagnosis and treatment of heart disease, further investigation is still needed. The availability of huge amounts of medical data leads to the need for powerful data analysis tools to extract useful knowledge. There is a huge data available within the healthcare systems. However, there is a task of effective analysis tools to discover hidden relationships and trends in data. Knowledge discovery and data mining have found numerous application in business and scientific domain. Researchers have long been concerned with applying statistical and data mining tools to improve data analysis on large data sets. Disease diagnosis is one of the applications where data mining tools are proving successful results. This research paper proposed to find out the heart diseases through data mining, Support Vector Machine (SVM), Genetic Algorithm, rough set theory, association rules and Neural Networks. In this study, we briefly examined that out of the above techniques Decision tree and SVM is most effective for the heart disease. So it is observed that, the data mining could help in the identification or the prediction of high or low risk heart diseases.
Separation of Judiciary and Judicial Independence in Bangladesh: An Appraisal  [PDF]

Abstract: Judiciary is not only confined to be an organ like executive and legislature but also something more. It is the last hope and aspiration to restore the rights of citizen in a country. But this judiciary cannot act to restore these rights unless and until it is free from any undue influence and interference of any other organ. Therefore, it is an obvious need that the judiciary should be separated in the true sense to perform its functions independently which is the main object of the judiciary. And where there is no effective separation of judiciary, there is no independent judiciary and where there is no independent judiciary, there is no rule of law. But mere separation is not enough to perform its function effectively. This paper will analyze the terms separation of judiciary and judicial independence and evaluate their proper implementation in Bangladesh and also some other factors which are essential with the separation of the judiciary to ensure independence. In Bangladesh, Judiciary has been separated from the executive in 2007. But due to some theoretical problems in justice system, the practice of executive interferences over judiciary is still continuing in Bangladesh in some context. “Judicial independence” is the concept that the judiciary needs to be kept away from the other branches of government. And the “separation of judiciary” is the pre-condition to judicial independence. To make concept of the research clear at first some terms should be made clear. Judicial independence refers to the separation of judiciary and independence of judiciary. Separation of the judiciary from the executive is established by the constitution of Bangladesh from its origin. But it was limited to mere documentary recognition before the decision of Masdar Hossain case in 2007 through which the separation of judiciary as well judicial independence was established in true sense. But it is not clear whether the judicial independence is actually implicated in proper way. To establish the proper judicial system, the separation of judiciary is a vital issue.

Abstract: The meltdown of financial markets across the world is one of the most common characteristics of modern capitalist economy. Starting with the stock market crash of 1901 through the great depression of 1930s to the Black Monday of 1987, the Asian financial crisis of 1997 and finally the latest tsunami of global financial crisis, the history of major stock market crashes are undoubtedly a testimony of its nature. According to IMF, the world has witnessed more than 100 stock market crisis in last 30 years. However, with the endeavors of so many talented academicians, policymakers, and researchers the reasons behind these crashes still remain unexplored. After almost 25 years nobody has a solid answer to the reasons behind stock market crash in 1987. Meetings after meetings, summits after summits, conferences after conferences of the world leaders, thinkers and academicians in the form of DAVOS summit, G-8 summit, EU summits, G-20 summits etc. have failed miserably to identify the root causes behind the crisis. At times the expectations of finding a common solution to the problem from such high level summits have ended up with common blame-games while the crisis continues to multiply as various economies enters into much painful long term recessions with human stories of sufferings, job loss and even suicide. Therefore, this article aims at uncovering the root causes of repeated form of global financial crisis by critically investigating the philosophical and functional foundations of existing system of financial economy. A detailed review of fundamentals, methods and facts about capitalism clearly highlights the inevitability of such problem to exist in existing financial economic model in the light of Islamic economic system.
Graft Quality Control with the Traditional Roller Pump Flow Measurement in Comparison to the Transit-Time Flow Measurement  [PDF]

Abstract: Objective: Vein graft quality control had been traditionally done for more than 20 years with a roller-pump flow measurement (RPFM) in our institute until off-pump coronary artery bypass grafting was introduced as the standard technique of myocardial revascularization in the year 2009 in our institute. In this study, the RPFM method was compared with the Transit-time flow measurement (TTFM). Patients and Methods: From February to September 2005, 31 patients (5 females) undergoing CABG in our institute were included into the study. All the patients were operated with cardiopulmonary bypass. In those patients, 27 distal anastomoses were done with the left internal thoracic artery and 99 anastomoses were done with vein grafts. Those 99 vein graft were assessed both, with the RPFM method and TTFM method. Results: CABG could be safely performed in all cases. In all target vessels, the RPFM method overestimated bypass flow compared to the TTFM method, and simple linear regression revealed no correlation between the TTFM measurement and the RPFM measurement (r = 0.08, p = 0.21). There were two cases in which a graft problem could be detected only with TTFM method, even in our small patient cohort. Conclusion: The present study suggests that the TTFM method is clearly superior to the traditional RPFM method and we believe now that TTFM should be performed routinely in all CABG to improve patient care and quality of surgical results.
